Job Summary:
Biigtigong Nishnaabeg Home & Community Care program supports a continuum of care for health services responsive to the needs of the frail, elderly, disabled, convalescing, and palliative care community members. The Community Health Program mandate and focus is on community health promotion and injury/illness prevention. The Home Care Nurse/Assistant Community Health Care will divide their time between these two programs to assist individuals, families, and community to achieve the long-range goal in improving health outcomes. Reporting to the Home & Community Care Coordinator, the Assistant Health/Home Care Nurse will be the lead individual responsible for completing client assessments, care plans, implementing appropriate nursing care and evaluating outcomes. They will create, coordinate, deliver and evaluate community prevention programs. They will effectively collaborate, communicate, and liaise with other members of the Health & Social Service team, other Biigtigong Nishnaabeg program staff and other health care agencies within the region.

Preferable skills & Qualifications:

  • A bachelor’s degree in nursing from an accredited university or Registered Nurse with diploma and minimum 3 years’ experience in home care setting or Registered Practical Nurse with diploma and minimum 3 years’ experience in home care setting
  • Current College of Nurses of Ontario registration and eligible for membership with RNAO /LAP
  • Current Foot Care Certification – Basic & Advanced or willingness to obtain upon employment
  • First Aid and CPR/AED Health Care Provider level or willingness to obtain upon employment
  • A valid Ontario Class “G” Driver’s License and access to a reliable personal vehicle
  • A recent (within 3 months) Criminal/Police Records Check and Vulnerable Persons Check

Duties & Responsibilities:

Duties:
Home Care

  • Work in a manner respectful of First Nations culture, values and beliefs
  • Provide a holistic approach in health care to clients looking at physical, mental, spiritual and social needs
  • Provide prescribed treatment for clients as ordered
  • Provides foot care for common foot and nail disorders as well as to high risk clients within their scope of practice. Understands and implements safety/infection control measures when providing services and sterilizing equipment following best practice guidelines
  • Conduct client nursing visits and assessments using the nursing process
  • Implements best practice standards and addresses ethical issues in accordance with appropriate College requirements and legislation
  • Preparing a client care plan using a client/family centered approach
  • Provide client and family teaching regarding care taking into consideration individual learning styles
  • Deliver services in the home which will successfully meet the client’s needs
  • Provide an objective, non-judgmental approach to client needs
  • Maintain accurate client files utilizing c-EMR
  • Work with Home & Community Care team to review and approve client care plans
  • Attend regular client case reviews with all service providers, care givers, and family members
  • Advocate on behalf of clients by providing assistance in accessing health care, social workers, legal aide, and other services as required
  • Provide professional ordered nursing care in accordance with program policy, and relevant legislation
    (Regulated Professional Act) when required
  • Maintain accurate reports and submit in a timely manner – I removed e-SDRT

Community Health Care

  • Work with Community Health Nurse to organize and implement community workshops and presentation on health promotion and prevention issues
  • Assist Community Health Nurse with events such as Flu Vaccine Clinics
  • The Assistant Health/Home Care Nurse will actively research and become familiar with traditional beliefs and practices of Indigenous people and will support the use of these beliefs by community members

Perform other duties, such as

  • Provide guidance and supervision to volunteers working in the program
  • Provide supervision and mentorship to students as required
  • Provide respite care and personal care to clients on an occasional basis
  • Identifies ongoing needs of elders and persons with disabilities and acts as an advocate to meet those needs
  • Participate in meetings with Health & Social Service Advisory Committee or Chief & Council and other committees as requested
  • Participate in meetings and be actively involved in the Accreditation process
  • Responsible for attention to client safety by addressing client safety on a regular basis in community newsletters
  • Ensure a work life and physical environment that supports the safe delivery of care and service to clients
  • Attends annual safety training
  • Continues professional development through participation in reflective practice, development of an annual training plan and by actively seeking and attending training opportunities applicable and relevant to position
  • Maintains relevant up to date data and stats on programs and submits written monthly report to Health & Social Service Director & Home & Community Care Coordinator
  • Other duties as required
