Under the direction of the Home and Community Care Coordinator, the Personal Support Worker is responsible to assist persons with tasks of daily living and/or personal care by promoting their independence in their home and community for as long as possible. The Personal Support Worker must work as a member of the team and in a team environment. To contribute to the quality of life of individuals who live at home by promoting, dignity, social/emotional well-being, mobility, personal appearance, comfort and safety.

Key duties include:

  • Support individuals and their families and provide friendly visiting to clients
  • Observe and report changes in conditions to the Case Manager/Coordinator or RN
  • Provide personal care services such as dressing, personal hygiene (bathing, skin care, toileting, mobility, transferring, Routine Practices of Daily Living and R.O.M. exercises in accordance with the Homemaker and Nursing Services Act and the Regulated Health Care Provider Act
  • Perform light housekeeping duties such as laundry and Environmental Cleaning in accordance to the Homemaker and Nursing Services Act, Ministry of Health Ontario Guidelines and Best Practice Infection Control Standards
  • To plan and prepare meals including special diets as required by the service care plan
  • Responsible for attention to client safety by addressing client safety on a regular basis
  • Ensure a work life and physical environment that supports the safe delivery of care and service to clients.
  • Submit weekly Reports of Service to the Staffing Assistant Personal Care and Assisted Living Services

Knowledge, Experience, Skills and Abilities:

  • Knowledge, training, and experience in home care
  • Knowledge of health-related issues among Aboriginal peoples, Aboriginal Culture & language
  • Clear understanding of policies and procedures that outline behaviors to promote client safety
  • Excellent interpersonal & communication skills with the ability to motivate self & others
  • Ability to advocate with families and Health Councils for the elderly and disabled
  • Punctual, Reliable and able to work under minimal supervision


A successful candidate must possess the following:

  • A Personal Support Worker Certificate;
  • First Aid and CPR Certificate;
  • Clean Criminal Reference Check;
  • A Valid Class “G” Driver’s License
